ONEgeneration has a wide range of social service programs to support older adults living independently in their own homes: From a simple referral for a newly experienced problem, to a thorough in-home evaluation and individualized care plan for daily support – our expert staff can help. Among other services, we can provide assistance with bathing, maintaining a home, delivery of daily hot meals, grocery shopping, caregiver support and ongoing case monitoring.

Our goal is to facilitate and enrich the lives of our frail seniors and to provide support for their caregivers and families in the community. We are truly one generation working together!

CARE PLANNING SERVICES

Client Evaluation & Assessment - An initial visit and determination of client needs, strengths and service provisions.
Care Plan - An interdisciplinary team of experts will review the evaluation and assessment to create a care plan. When desired, ONEgeneration can act as an ongoing care manager to ensure plan goals are met.
Case Monitoring - Clients are reevaluated regularly to keep up with changes in physical or emotional status to determine the services needed to maintain independence.
CareLine - If desired, a daily morning check-in call to seniors can make sure all is well.
Emergency Alert Response System - An alert system is put in the client's home with a direct connection to police, fire and medical emergency services.
Assistance with Bathing - An aide assists client with bathing, shaving, brushing teeth, grooming and dressing.
Grocery Shopping - A volunteer will be scheduled weekly or bi-weekly to provide grocery shopping and delivery services.
Daily Hot Meal Delivery - One hot lunch delivered daily, Monday - Friday. Ask about our weekend delivery options.
Light Housekeeping -Assistance with household chores and maintenance are available.
Family Caregiver Support - ONEgeneration can help a family member cope with the needs of their frail loved one. Whether through information, referral, support groups, or placement assistance for long term care, ONEgeneration can meet your needs.
